If possible, it is best to avoid taking any medication during pregnancy (except for pre-natal vitamins). Some medications are safe during pregnancy while others are not. At times, your doctor may substitute safe medications for the unsafe ones if you are required to continue your treatment during pregnancy. It is unwise to discontinue medications on your own because doing so may lead to worsening of your lupus and cause damage to your baby. Selected medications are discussed below but you should always review your medication list with your physician during the pregnancy planning phase.

  • use of corticosteroids for prolonged periods or repeatedly in pregnancy is associated with intra-uterine growth retardation (IUGR) in the foetus
  • no evidence of an increased incidence of IUGR following short-term treatment, such as prophylactic treament for neonatal respiratory distress syndrome
  • no convincing evidence that systemic corticosteroids cause an increased risk of congenital abnormalities such as cleft lip or palate
  • prednisolone is excreted in small amounts into breast milk. However, systemic effects in the infant are unlikely with doses of up to 40mg of prednisolone or equivalent; infants should be monitored for adrenal suppression if the mothers are taking a higher dose

Some mothers-to-be stay in the hospital for the weeks prior to labor to ensure blood tests are monitored and normal, while others are capable of staying home if they’re condition hasn’t worsened. Either way, doctors need to keep a very careful eye on blood pressure levels, the mother’s and baby’s heartbeats, water retention, urine concentration, and other symptoms that can develop due to complications.
